Information & Assistance: Information given to a person of any age, that allows them to contact directly an aging service provider.

Outreach: Identify and contact elderly persons in need of assistance or services and to inform them of what is available. May include
referral and service arrangement.

Case Coordination & Support: Long-term client care support services focusing on person centered planning. Arrangements with services to allow seniors to remain living safely and independently in their own homes. Service Workers maintain regular contact with the client, review client needs and monitor services to ensure quality and effectiveness.

Western Township Outreach: Case Workers Services are available at Republic, MIchigamme, Champion & Humboldt township
sites. Funded in part by United Way of Marquette County

Homemaking: Help with everyday housekeeping tasks such as light cleaning, meals, and laundry that are difficult or dangerous for a senior to perform. May include short errand running if qualified.

Chore: Minor safety related home repairs and routine home chores for those who are unable.
Includes such tasks as grass cutting, window washing, smoke alarm installation, and snow shoveling.

Financial Management: Assistance with paper work for state, federal tax and tax credit forms, insurance reimbursement, medical & prescription plans, food programs, household management and other financial concerns.

Other Case Management Services: Help is provided to seniors, family members, or caregivers who are experiencing difficulties in coping with personal problems, aging issues, illness, family relationships, and other concerns. Specialized visits and arrangements for services to older persons experiencing difficult psychological adjustments or those at risk of needing more intensive clinical or  hospital services without intervention.

Caregiver Connect: second Monday of the month, 1:30-3PM. Provides useful and needed information and peer support for those providing care to others and for selfcare.

Health Screenings: Free Walk-in Blood Pressure Clinic Plus (free blood sugar screening
and $5 blood cholesterol checks), 3rd Friday –11 to Noon.

Monthly Foot Care Clinic by appointment, provided by U.P. Home Health at a cost of $25.

Newsletter: Articles of interest, calendar of center and other senior events, monthly menu of the senior nutrition meal program, and other items of interest. The Skylight is mailed the beginning of each month and is available at the senior center and at other locations. Call 485-5527 to be added to our mailing list.

Senior Meal Program: The Senior Center is a Senior Nutrition Noon Meal congregate site. You must call a day ahead or sign a meal res-ervation sheet posted in the center to receive a meal. This program is provided through the Community Action Alger-Marquette. Suggest-ed donation is $3.50 for those sixty years and older and $6.50 for those under sixty years of age. The Senior Center Case Workers make referrals to the homebound meal pro-gram for those qualified to receive this ser-vice.

Thursday Noon Special Meals: Menu post-ed in newsletter and Facebook. Must make reservation day before meal. - suggested $6 donation.

Free Legal Clinic: The second Monday, at 9 am, Free Walk-in Clinic to help with general legal concerns and those of special interest to seniors.
